Kindergarden Registration

Registration for ALL students in kindergarten through eighth grade will be held from 8:00 A.M. to 6:00 P.M. on Monday, August 6th and from 8:00 A.M. to 1:00 P.M. on Tuesday, August 7thRegistration will be conducted at the school in which the student is enrolling Children do not need to be present during registration.  Parents who are unable to register their children on either of these two days should call their child's school after August 8th between 8:00 A.M. and 4:00 P.M. to make other arrangements.

All district registration forms can be completed and printed prior to registration by accessing the district website at http://choosestyle.fes.org and clicking on the 2007-08 District Registration Forms link located on the right hand side of the screen.  If you do not have access to the Internet, blank district forms will be available to complete at each registration site.

Only children who will be five years old on or before September 1st are eligible to enroll in kindergarten.  Likewise, only children who will be six years old on or before September 1 are eligible to enroll in first grade.  Full-day and half-day kindergarten programs are available to all students.

Parents who have pre-registered their child for kindergarten must complete the registration process [pickup parent handbooks and school supply lists, pay fees ($40.00 for Elementary and $15.00 for Middle School), return their child's school physical and immunization record, apply for free or reduced lunch, meet school officials, etc.] on either of the above dates.  All parents should bring their children's Social Security numbers and the name and telephone number of an adult to contact in case of an emergency during school hours.

All kindergarten, fifth grade, ninth grade, and out-of-district transfer students are required to have had a physical examination within the previous 365 days, and to be properly immunized.  Any students in special education programs that will have turned, or will be turning, ages five, ten, and fifteen in 2007 are required to have a physical examination and to be properly immunized. All kindergarten, second, and sixth grade students must present proof of having been examined by a licensed dentist before May 15th of the current school year.  Parent(s)/Guardian(s) of children between the ages of 6 months and 6 years must provide a statement from a physician that their child was "risk assessed" or screened for lead poisoning in accordance with State Law.

The school nurse will be available during registration to check for compliance.  Students who are not in compliance will not be allowed to attend school.  However, recent out-of-state transfer students who did not attend the previous school year may request an extension of this deadline from the school nurse.  In addition, an official copy of the child's birth certificate (not a hospital certificate) must be presented at the time of registration for a child enrolling for the first time in the district (kindergarten or out-of-district transfer).  Students will not be allowed to attend school without first showing proof of a valid physical and immunizations.

Parents will be required to provide evidence that they reside in the School District by showing a valid driver's license or some other proof of residency.  All parents will be required to fill out a residency questionnaire.  Parents who are divorced and non-parent guardians must provide a copy of their divorce decree or court order granting them legal custody.  Only resident students of the Choosestyle School District may attend the Choosestyle Schools without the payment of tuition.  Any person who knowingly or willfully gives any false information to a school district about the residency of a student in order to avoid tuition maybe guilty of a Class C misdemeanor.

Transportation personnel will be available to answer questions about bus routes, schedules, and assignments.  Children who reside less than one and one-half miles from school are not eligible for bus transportation.  Pick-up and drop-off at a childcare provider is available only on established bus routes for eligible riders.

Choosestyle School District allows a limited amount of student transfers between elementary attendance centers.  Requests to transfer between elementary schools must be made in writing to the building principal.  Approval of such requests will be made solely on the basis of class enrollment that, in some cases, will not be known until after school begins.  Transfer Application forms will be available at registration.

Students from families that meet certain income requirements are eligible for free or reduced price meals.  Information on eligibility guidelines will be available during registration.

All school offices will be open from 8:00 A.M. to 4:00 P.M. on August 1st, and thereafter.  School principals will be available to answer questions or provide information concerning educational matters.
